Registration

On this page you will find all of the necessary documents to start your child's enrollment process at

Harmony Preschool Ltd.

The first step is to fill out the Registration form and pay your registration fee.  Then you must complete the State forms and get the medical form signed by your doctor with an immunization record.  If your child has an EpiPen or Inhaler you will need to complete the bottom two forms also.
